The best central source for product safety advisories in the U.S. is the U.S. Consumer Product Safety Commission http://www.cpsc.gov their site can be searched by product type or company. Not all recalls involve the CPSC. If multiple vendors utilize a recalled component in their gear, all vendors names may not be listed, it is best to inspect your gear for use of the component. Date signifies recall notice publication, not product sale date

ROMI Recalls SCUBA Regulators Due to Drowning Hazard

Y An internal component that seals air between the high pressure first-stage and the intermediate pressure second-stage can fail. This can result in uncontrolled flow of air to the diver and pose a risk of serious injury or death.
ROMI has received two reports of units malfunctioning during diving and six reports of units malfunctioning while being tested by dealers prior to delivery to consumers. No injuries reported.

M&J Engineering Recalls Swivels for Scuba Diving Masks

Y AGA Swivels for Scuba Diving Masks. The swivel, which is attached to a diving mask, could separate while diving. This will result in a sudden loss of the diver’s air supply, causing the diver to engage in emergency ascent.
This recall involves all SW-3000 2nd stage 360-degree swivels for scuba diving masks manufactured between January 2004 and February 2005. The swivel has the date stamped on its body, below the patent number. Swivels that are already repaired have an arrow stamped on the top right corner of the swivel body.

Y The swivel, which is attached to a diving regulator, could separate while diving. This will result in a complete and sudden loss of the diver's air supply, causing the diver to engage in emergency ascent procedures.
The recall involves the HO110 Swivels sold as an aftermarket regulator component. There are no markings such as a date or production code embossed on the unit. Contact Innovative Scuba Concepts for information on where the recalled units were sold.

Scubapro® Recalls MK 20 Scuba Regulators for Service

October 2, 2006
Y Over-tightening of the Yoke or Din retainer during annual servicing of these regulators could result in a stress crack and failure of the regulator. If this occurs during a dive, air supply could be interrupted, posing a drowning hazard.
Scubapro® USA has been providing a service upgrade kit via the authorized dealer network since May 2005 which contains a Yoke or Din retainer that prohibits over-tightening during annual service. Consumers can easily identify if the service upgrade has been completed by counting the 2vents on the plastic saddle located on the regulator. If the service upgrade has not been completed (3 vents), the consumer should bring the regulator to any authorized Scubapro® dealer to have the service upgrade completed free of charge.
